Report your child’s vaccines!

This fall, your child will need proof of vaccination, or a valid exemption, for the following diseases:

Tetanus, Diphtheria, Polio, Mumps, Measles, Rubella

  • NEW Meningococcal disease
  • NEW Pertussis (whooping cough)
  • NEW Varicella (chickenpox) - required by children born in 2010 or later

If your child has followed the current Publicly Funded Immunization Schedule for Ontario , they should be up-to-date for school attendance. This schedule can be found on your child’s yellow immunization record or at Ottawa.ca/immunization. Make sure that all vaccines given to your child by your healthcare provider are reported to Ottawa Public Health. You can update your child’s record online. Ottawa Public Health requires proof that your child has received proper immunization. All children attending school in Ontario must be immunized against diphtheria, tetanus, polio, measles and rubella, unless there is a valid exemption.
